Essential Job Functions:
- Managing the day-to-day operations of the practice.
- Greet patients who enter the facilities with warmth.
- Assist/Interact with patient arrival and basic appointment setup and preparation – may include basic vitals and blood draws.
- Address any patient concerns or complaints.
- Organize and maintain patient records using electronic medical records.
- Answer and make phone calls.
- Monitor inventory and place orders for resupply (office and medical).
- Negotiate with service providers, suppliers, and medical sales representatives.
- Coordinate scheduling for medical providers.
- Assist with billing and collections as needed.
- Assist with reconciliation of accounts as needed.
- Write lab orders.
- Submit authorizations.
- Work as a team to ensure the practice runs effectively and meets objectives
- Assist with additional office management coverage and tasks as needed.
Qualifications/Knowledge/Skils/Abilities:
- High school graduate
- Two (2) years of college education preferred.
- CMA/LPN certification
- 3 – 5 years in an administrative/support capacity in a health care setting.
- Excellent customer service with a client focus.
- Computer literacy: familiarity with Electronic Medical Record systems, Office 365 and Windows based software applications including but not limited to Outlook, Word, Excel, MSTeams
- Ability to handle a variety of tasks and responsibilities at any given time and be flexible to changes brought about by refocused priorities.
- Knowledge of basic bookkeeping, organizational policies, procedures, and systems.
- Ability to operate and perform basic troubleshooting of standard office equipment (copier, fax, personal computer, printer, etc.)
- Ability to prioritize and organize projects effectively.
- Capable of quick and effective problem solving.
- Uses independent judgment, prioritizes, and demonstrates decision making skills. Able to seek assistance and guidance from others when necessary.
- Displays excellent written and verbal communication skills.

The Nurse Practitioner is responsible for providing home-based medical care to patients in their residences and assisted living facilities in a defined geographic area. Provide medical care to patients in institutions such as nursing homes. The provider will work collaboratively with the medical director and other care team members to ensure the best outcome aligned with the goals of care for the patients.
Essential Functions:
- Assesses health status by obtaining relevant health and medical history. Perform physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician. Identify significant health and medical problems and provide intervention for each as needed.
- Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient’s clinical problems and health care needs. Develop a diagnostic plan by analyzing and synthesizing collected data.
- Perform diagnostic or therapeutic interventions consistent with certification, law, and Health System privileges. May prescribe and monitor medications as allowed by prescriptive authority through Virginia Board of Nursing and Health System policy.
- Develop a treatment plan in collaboration with the patient, significant others, and the AIC IDT. Participate in IDT meetings and collaborate with nursing, medical, and other healthcare team members to ensure quality patient care.
- Refer patients to specialty providers as appropriate for consultation or specialized health resources and treatment. Ensure continuity of care by serving as a liaison between the patient and other members of the multi-disciplinary care team or with other specialty areas as necessary.
- Perform phlebotomy, remove cerumen, and other procedures such as I&D, debridement of wounds, suture/staple removal, and minor laceration repairs.
- Implement the plan of care and evaluate patient status, specifically identify and manage geriatric syndromes including but not limited to frailty, age related physical debility, memory loss, depression, and falls. Determine the effectiveness of the plan of care by documenting patient clinical outcomes. Modify the plan as necessary to achieve health and medical goals.
- Develop patient education plans. Provide health education and utilize community resources as appropriate to promote optimal health.
- Participate in quality monitoring through the review of records and treatment plans for patient outcomes periodically. Apply standardized care guidelines in clinical practice as applicable.
- Initiate and/or collaborate in performing clinical research projects, including retrospective and prospective study registries and the preparation and presentation of information.
- Demonstrate a high standard of moral and ethical behavior. Demonstrate compassion, professionalism, and commitment to excellent patient care.
- Assist in the development of nurse practitioner clinical protocols for new eldercare initiatives.
- Participate in community outreach initiatives.
- Participate in professional development activities and maintain professional licenses and CE requirements.
- Communicate with home health nurses, care coordinators, other providers, and DME suppliers to optimize care at home.
- Provide documentation necessary to obtain medications, home health care, hospice care, and DME.
Education:
- Required: Master's or doctoral degree as a Nurse Practitioner. Three years current Nurse Practitioner in an outpatient or long-term care setting.
- Preferred: Completion of a gerontological nurse practitioner master program. Previous experience in primary care, geriatric care, palliative or hospice care.
Additional Eligibility Qualifications:
- Advanced Practice Registered Nurse with current Virginia license
- National certification as Nurse Practitioner required with Gerontological certification (GNP-BC) preferred. Alternate certifications could include Adult Nurse Practitioner Certification (ANP-BC) or Adult Gerontology Primary Care Nurse Practitioner Certification (AGPCNP-BC)
- DEA registration to prescribe preferred medications/interventions
- CPR Certification
- Valid Virginia Driver's license upon hire
- Proof of automobile insurance coverage upon hire

Qualifications:
- Bachelor’s degree required in an area with major focus on counseling, such as social work, counseling, psychology, or pastoral counseling. MSW or license eligible preferred.
- At least one year experience in counseling or education in grief or loss counseling (or related field).
- Excellent communication and telephone skills.
- Emotionally mature and capable of dealing with dying and bereavement individuals.
- Able to function independently.
- Flexible schedule to allow for evening support groups and community presentations.
- Valid driver’s license, reliable transportation, current motor vehicle insurance, and driving record per hospice policy.
Essential Job Functions:
- Works with Interdisciplinary Team (IDT) to identify bereavement risk factors and develop a bereavement plan of care.
- Incorporate information gathered from the patient’s initial assessment and bereavement assessments into the bereavement plan of care.
- Conduct bereavement assessment in accordance with the appropriate level of care.
- Make bereavement calls, visits, and offers to provide bereavement services. Provide follow up support as needed. Document in the client’s electronic record.
- Work with bereavement volunteers and make assignments as appropriate.
- Refer clients to grief counselor and community resources, including support groups, as appropriate.
- Serve as resource on grief and loss issues to staff. Provide short-term assistance to staff as needed.
- Assist supervisor in developing/revising bereavement services policy as needed.
- Facilitate hospice grief support group meetings as assigned.
- Provide community education programs about grief and loss.
- Work with IDT to coordinate the Rose Program at the time of death sending of sympathy cards and mailings to ConnectionsPlus families as directed.
- Provide bereavement counseling to hospice family members after a patient’s death as requested.
- Provide pre-bereavement counseling support to family members as requested.
- Perform chart reviews and Quality Assurance and Performance Improvement (QAPI) activities as directed.
- Participate in training/continuing education about grief/loss issues.
- Provide bereavement education and support to the staff of facilities that have cared for our patient, upon request.
- Coordinate annual memorial services.
- Perform other tasks/duties as assigned.
