ConnectionsPlus Healthcare + Hospice, formerly Rockbridge Area Hospice and Mountain Regional Hospice, is looking for Social Worker/Counselor/Care Navigator/Death Doula to join our growing integrated healthcare team.

This role functions as part of a team working with individuals and families as they deal with declining health, providing families and patients psycho-social-spiritual support through education, resource connection and counseling. Issues addressed include end-of-life planning, caregiving, anxiety and depression, and grief. This role is an integral part of our organization providing compassionate care
and comfort to our neighbors living with serious illness so they and their families can enjoy life for as long as possible.

- 9 hours of PTO accrued per pay period (starts on first full pay period)
- 0% cost paid by employee for medical insurance (other plans available for dependents)

- Mileage reimbursement
- Monday-Friday 8:30am - 5:00pm
- Part-time employment may be considered.
