Hospice care is specialized comfort care for patients with a life-limiting illness. ConnectionsPlus provides pain management, emotional support, spiritual care, and family counseling — at home or in a care facility. Our 5-star rated team is available 24 hours a day, 7 days a week.
Hospice may be appropriate when a patient has a terminal illness with a prognosis of six months or less. Signs include frequent hospitalizations, significant weight loss, increasing pain, and declining function. ConnectionsPlus offers free consultations to help families decide what is right for their loved one.
