We will ask for:
At that point, our Admissions team will contact the patient and/or family to establish a time for a consult. We’ll review the patient’s medical history and apply our prognostics as we assess their current condition, priorities and wishes.
Once we make a determination and come to an agreement with the patient and their caregivers, we will touch base with you for further steps as we develop a plan of care. You remain part of our patient care team while we provide the palliative and end-of-life services we specialize in.
We will ask for:
- Patient's name, address, phone number and date of birth
- Diagnostic presentation
- Your thoughts on "why hospice. why now"?
- Information about any conversation you've had with them about end-of-life plans/needs
