If you think that a patient may be eligible, call us. We will consult with you and the patient & their family to help make the determination.
We use research-based prognosticators to help determine appropriateness for the hospice level of care. These include:
Individuals eligible for our services may experience some or all of the following:
State and Federal regulations require that the following two criteria be met for a patient to be eligible for the hospice benefit:
Our patients can receive care in their own home or a family member's home, an assisted living facility, or a nursing home.
We use research-based prognosticators to help determine appropriateness for the hospice level of care. These include:
- MDS Mortality Risk Index (MMRI-R)
- Functional Assessment Staging Tool (FAST)
- Palliative Performance Scale (PPS)
Individuals eligible for our services may experience some or all of the following:
- Frequent hospitalizations
- Progressive weight loss
- Deteriorating mental abilities
- Recurrent infections
- Decreased ability to perform Activities of Daily Living (ADLs)
State and Federal regulations require that the following two criteria be met for a patient to be eligible for the hospice benefit:
- Two physicians must confirm the patient’s life expectancy is 6 months or less, if the illness runs its normal course (One of these is our Medical Director).
- Patient has chosen care that focuses on symptom management rather than curative treatment. (Quality of life overrides Quantity of days)
