Together with patients and families, we create a personalized Plan of Care that reflects individual needs, values, and wishes. Our care includes expert pain and symptom management, emotional and spiritual support, and grief counseling for loved ones. |

- Improved Quality of Life: Hospice helps manage symptoms and provides holistic support.
- Care at Home: Patients and families who prefer to receive care in the comfort of home—wherever that may be—can avoid unnecessary hospitalizations.
Our Hospice Team
At ConnectionsPlus, our interdisciplinary team works together to provide compassionate, comprehensive care:
- RN Case Managers: Coordinate care plans, manage symptoms, and ensure comfort.
- Nurse Practitioners: Provide medical oversight and continuity of care.
- Certified Nursing Assistants (CNAs): Deliver hands-on personal care with dignity and respect.
- Social Workers: Offer emotional support and help navigate care transitions.
- Chaplains: Provide spiritual care tailored to each person’s beliefs and values.
- Volunteers: Offer companionship, caregiver respite, and help with everyday tasks.
