Hospice with ConnectionsPlus

What Is Hospice?
Hospice is not a place—it’s a philosophy of care focused on comfort, dignity, and quality of life for individuals with a life-limiting illness. At ConnectionsPlus Healthcare + Hospice, we believe in helping patients live fully and meaningfully, even as they approach the end of life.
Whether you live in a private residence, with a loved one, in assisted living, or a skilled nursing facility, our hospice team comes to you--wherever you call home. Hospice care does not seek to hasten or prolong life but instead supports patients in doing what brings them peace, joy, and fulfillment.
We treat the person, not the disease. Together with patients and families, we create a personalized Plan of Care that reflects individual needs, values, and wishes. Our care includes expert pain and symptom management, emotional and spiritual support, and grief counseling for loved ones.

Who Is Eligible for Hospice?
To qualify for hospice services:
  • The patient has a life-limiting illness where a cure is no longer likely.
  • The focus of care has shifted from curative treatment to comfort and symptom management.

When to Consider Hospice
  • Early Support: The sooner hospice is considered; the more benefits patients and families can receive.
  • Improved Quality of Life: Hospice helps manage symptoms and provides holistic support.
  • Care at Home: Patients and families who prefer to receive care in the comfort of home—wherever that may be—can avoid unnecessary hospitalizations.

Our Hospice Team
At ConnectionsPlus, our interdisciplinary team works together to provide compassionate, comprehensive care:
  • RN Case Managers: Coordinate care plans, manage symptoms, and ensure comfort.
  • Nurse Practitioners: Provide medical oversight and continuity of care.
  • Certified Nursing Assistants (CNAs): Deliver hands-on personal care with dignity and respect.
  • Social Workers: Offer emotional support and help navigate care transitions.
  • Chaplains: Provide spiritual care tailored to each person’s beliefs and values.
  • Volunteers: Offer companionship, caregiver respite, and help with everyday tasks.
  • Care Navigators: Guide families through the hospice journey with ongoing support.

We’ll be there when you need us. As a patient of ConnectionsPlus, you are in control of the care you receive. Our team is here to support you--whenever, wherever, however.
