Our Dementia Care Management Program is a compassionate, community-based service focused on upholding the dignity, safety and well-being of people living with dementia — while also supporting the caregivers who walk this journey alongside them.
We recognize that dementia affects not just the individual, but the entire family. That’s why our program offers a unique, whole-family approach that integrates medical, emotional, and practical support. Whether you're just beginning to notice signs of memory loss or navigating the complexities of long-term care, we’re here to help — every step of the way.

Who We Serve
  • Individuals living with dementia seeking reassurance, safety, and dignity.
  • Caregivers and families hoping for emotional support, and practical tools to sustain care at home.
  • Primary care providers looking for coordinated, comprehensive support for their patients and families.
Our Goals
  • Provide early, ongoing support — long before more intensive care is needed.
  • Empower caregivers with education, resources, and respite to reduce stress and sustain connection.
  • Ensure individuals with dementia receive person-centered care that honors their identity, preferences, and safety.
  • Build long-term relationships with families, offering guidance and connection throughout the dementia journey.
  • Act as a bridge to community resources, aging services, and healthcare providers.
