Our Dementia Care Management Program is a compassionate, community-based service focused on upholding the dignity, safety and well-being of people living with dementia — while also supporting the caregivers who walk this journey alongside them.
We recognize that dementia affects not just the individual, but the entire family. That’s why our program offers a unique, whole-family approach that integrates medical, emotional, and practical support. Whether you're just beginning to notice signs of memory loss or navigating the complexities of long-term care, we’re here to help — every step of the way.
